AI tools in neurosurgery: what's being used in 2026
By the Babels.AI team · · 7 min read
Artificial intelligence in neurosurgery has moved from research papers into everyday hospital workflows. The most widely used tools today read brain scans to flag strokes and bleeds within minutes. Others analyze tumor tissue during surgery, guide robots and navigation systems, predict outcomes, and, in clinical trials, translate brain signals into movement and speech. Here is what each category does and which tools lead it.
What is AI used for in neurosurgery?
AI in neurosurgery falls into six main groups:
| Category | What it does | Examples |
|---|---|---|
| Stroke and bleed detection | Flags emergencies on CT scans and alerts the team | Viz.ai, RapidAI, Aidoc, Brainomix |
| Tumor imaging | Finds, outlines and measures tumors on MRI | Research and commercial segmentation tools |
| In the operating room | Reads tissue, tracks the brain, guides instruments | Stimulated Raman histology with AI, navigation, robots |
| Outcome prediction | Estimates an individual patient's risks | Risk calculators and machine learning models |
| Brain-computer interfaces | Turns brain signals into actions or speech | Neuralink, Synchron, Precision Neuroscience, Paradromics, Blackrock Neurotech |
| Documentation and reference | Drafts notes and answers clinical questions | Ambient AI scribes, medical search assistants |
How does AI stroke detection work?
AI stroke detection software analyzes CT scans as soon as they are taken and alerts the stroke team if it finds a likely large blood vessel blockage or a brain bleed. Speed matters because treatments that remove a clot work best when done quickly.
In 2018 the FDA authorized Viz.ai's software as the first AI tool of its kind to triage suspected large vessel strokes, and the field grew quickly from there. The main products today:
- Viz.ai analyzes CT angiography for large vessel occlusions and sends alerts with images to the care team's phones. It is used in a large share of U.S. stroke centers.
- RapidAI began with perfusion imaging, which estimates how much brain tissue is at risk, and now offers detection of blockages and bleeds.
- Aidoc offers a platform of FDA-cleared algorithms across radiology, including brain bleeds and vessel blockages.
- Brainomix offers stroke imaging software that is widely used in the UK and Europe.
These tools do not make the diagnosis. A radiologist and the stroke team review every case. Their value is in getting the right specialist's attention to the scan sooner.
Can AI detect brain tumors?
Yes. AI can detect and outline brain tumors on MRI, and it is increasingly used to measure them. Deep learning models are very good at segmentation: drawing the boundary of a tumor on each MRI slice. That makes it faster to measure tumor volume, track growth between scans, and plan surgery or radiation.
Research models can also estimate a tumor's type or genetic features from imaging alone, but these are not a substitute for a tissue diagnosis. In practice, AI on MRI is most useful for measurement and consistency, while the diagnosis still comes from a biopsy or surgery.
How is AI used during brain surgery?
The newest AI tools work inside the operating room.
Reading tumor tissue in minutes. Traditionally, a sample taken during surgery is sent to a pathology lab, and the result takes some time. Stimulated Raman histology uses lasers to create microscope images of fresh tissue right in the operating room. Paired with AI, it can predict a tumor diagnosis within a few minutes. Teams at the University of Michigan and UCSF developed FastGlioma, an AI model that analyzes these images in seconds to estimate how much tumor is left at the edge of a resection, so surgeons can decide whether to remove more.
Tracking brain shift. The brain moves during surgery as fluid drains and tumor is removed, which makes navigation based on pre-operative scans less accurate. This is called brain shift. Machine learning models that compare intraoperative ultrasound sweeps taken before and after resection can estimate that movement.
Navigation and robotics. Navigation systems from companies such as Brainlab and Medtronic track instruments against the patient's scans. In spine surgery, robotic systems such as Medtronic's Mazor and Globus Medical's ExcelsiusGPS help place screws along a planned path, and augmented reality headsets project the plan onto the surgeon's view. These systems follow the surgeon's plan and work under the surgeon's control.
Can AI predict surgical outcomes?
Yes. Prediction models estimate an individual patient's risk of outcomes such as complications, tumor recurrence, a treated aneurysm filling back in, or degeneration next to a spinal fusion. Older risk calculators used simple statistics. Newer machine learning models can capture more complex patterns, especially for outcomes that unfold over time.
Good prediction tools share a few features. They report how well they rank patients (the C-index), whether their probabilities can be trusted, and which factors drove each prediction, often using SHAP. We publish several research examples of this kind of model, such as a recanalization risk calculator for coiled aneurysms.
What is a brain-computer interface?
A brain-computer interface (BCI) is a device that records brain activity and uses AI to translate it into commands, such as moving a cursor, typing text or producing speech. Current BCIs are designed for people with paralysis or conditions such as ALS, and they remain in clinical trials.
The leading companies take different approaches:
- Neuralink implants a small device with very thin, flexible threads of electrodes into the brain using a surgical robot. Its first human participant was implanted in early 2024, and trials have since expanded to more participants and countries.
- Synchron places its Stentrode inside a blood vessel next to the brain through a vein, avoiding open brain surgery. It is among the furthest along toward a larger pivotal trial.
- Precision Neuroscience places a very thin film of electrodes on the brain's surface and has received FDA clearance for temporary use of its electrode array.
- Paradromics is developing a high-channel implant focused on restoring speech.
- Blackrock Neurotech makes the Utah array, used in academic BCI research for many years.
Is Neuralink FDA approved?
No. Neuralink is not FDA approved for general use. It is being tested in clinical trials under FDA permission to study the device, which is different from approval to sell it. The same is true of the other implanted BCIs designed for long-term use.
What are the best AI tools for doctors?
Outside imaging, two categories have spread fastest:
- Ambient AI scribes listen to the visit (with consent) and draft the clinical note, cutting documentation time.
- AI medical search assistants answer clinical questions with references to the medical literature.
Both save time, but their output must be checked. Drafted notes can contain errors, and AI answers can misstate evidence.
What are the risks and limits of AI in neurosurgery?
- Validation. A tool that performs well in one hospital can perform worse in another with different scanners or patients.
- Bias. Models learn from past data and can perform unevenly across patient groups.
- Over-reliance. Alerts and predictions support decisions; they do not replace the clinical exam and judgment.
- Prediction is not causation. A risk model can identify who is at higher risk without proving that changing a treatment will help.
- Regulation. FDA clearance covers a specific use. A tool used outside that use has not been evaluated for it.

Frequently asked questions
Is Viz.ai FDA approved?
Viz.ai's stroke software is FDA cleared. In 2018 it received the first FDA authorization for AI software that triages suspected large vessel strokes on CT scans, and the company has received additional clearances since.
What is the difference between Viz.ai and RapidAI?
Both detect strokes on CT scans and alert care teams. RapidAI is especially known for perfusion imaging that estimates how much brain is at risk, while Viz.ai is known for its care-team communication platform. Many hospitals choose based on workflow and which scans they rely on most.
Can AI replace neurosurgeons?
No. AI tools help detect, measure, predict and guide, but they do not perform brain surgery or make treatment decisions. Robotic systems in neurosurgery follow a plan set by the surgeon.
What is a brain-computer interface in simple words?
It is a device that reads signals from the brain and uses a computer to turn them into actions, such as moving a cursor or speaking words. Today it is mainly being tested to help people who cannot move or speak.
How accurate is AI at detecting brain tumors?
AI is very accurate at outlining tumors on MRI and is used to measure them. Diagnosing the exact tumor type still requires tissue from a biopsy or surgery, although AI can now help analyze that tissue in the operating room within minutes.
